Childcare check-in will begin 15 minutes prior to event start time.

Event End Time

We request childcare pick-up not be past 8:30p.m. (Exceptions for Fri/Sat night special events.)

Estimated Number of Children Attending

Will a meal be provided for the children?

We will provide a light snack. If you are providing dinner for the adults, please make sure to provide a dinner for the children.

Please include any other information you would like us to know about the event (i.e. offsite, special needs, etc.)
