CHILDCARE Request Form

Please note: Childcare will be available only for children ages 6 months - 5th grade. We would appreciate at least 2 weeks notice prior to your event to allow us to prepare for your childcare needs. Once you fill out this form, we will let you know if your childcare request has been approved. If you have not heard back from us, please feel free to email us at childcare@ridgeview.net to check on the status of your childcare request. Thank you!
Please include start date and end date if you are requesting continuous care for several days or weeks, such as for a recurring Bible study.
Childcare check-in will begin 15 minutes prior to event start time.
We request childcare pick-up not be past 8:30p.m. (Exceptions for Fri/Sat night special events.)

We will provide a light snack. If you are providing dinner for the adults, please make sure to provide a dinner for the children.
